Top priorities in all healthcare organizations are improving patient safety and the patient experience. Nationally, The IOM published concerns with fragmented processes causing harm. The Joint Commission highlighted ineffective communication as a factor in medical errors. It is crucial for leaders to promote
evidence based practices to improve patient outcomes.
