The effectiveness of telecare service on caregiver burden, mastery of stress, and family function among family caregivers of heart failure patients: Quasi-experimental study design

Purpose: To evaluate the effectiveness of nursing care based on the Telecare Service (TCS) on family caregiver's burden, mastery of stress, and family function among family caregivers of patients with heart failure.
Methods: Sixty families of patients with heart failure were recruited in a medical center during 2010 May to Oct. Thirty of them who participate the TCS after discharge from the hospital to home were allocated as the experimental group; the other thirty families were only participate the discharge planning was the comparison group. The family caregivers of both groups received the discharging planning about caring patients with heart failure. Data of caregiver burden, mastery and family function were collected before discharge planning at hospital and one month later at home. Effects of group, time, and group-time interaction were analyzed using Mixed Model in SPSS (17.0).
Results: Family caregivers in both groups have significantly lower burden and higher mastery, and better family function. Caregiver burden was significantly lower for the family caregivers in the experimental group than in the comparison group. Mastery of stress and family function were significantly higher for the family caregivers in the experimental group than the family caregivers in comparison group.
Conclusion: Telecare service with discharge planning could reduce family caregiver burden, improving their mastery of stress, and thus increasing their family function. The results provide the evidence that nurses providing discharge planning for family and Telecare nurses followed-up their health condition with Tele-Care system by telephone shorten the distance barriers to providing the necessary family nursing intervention could help the family of patients with heart failure successfully transition from hospital to home.
