Heart failure (HF) is a common end-stage disease of many cardiovascular diseases. It has a high rehospitalisation and mortality rate, with a third of patients dying within one year of discharge. My projects have been translated into clinical practice which have had a direct benefit on reducing hospitalisations and mortality.
