One academic university created interprofessional didactic and clinical learning experiences for students within the Schools of Nursing, Social Work, Health Professions–Rehabilitation Counseling, and Pharmacy that resulted in improved models for patient care delivery. The nurse-led FQHC uses the model and has implemented innovative approaches to engage/keep patients in care.
